The Chronic Disease Program is part of the Nursing Division of the Vanderburgh County Health Department.  Our staff of registered nurses are available Monday through Thursday, 8:00AM to 3:30PM.  The objective of this program is to reduce the incidence of disease and death in Vanderburgh County related to hypertension, cardiovascular disease, diabetes, cancer, and renal disease.

HYPERTENSION:hypertension

  1. Blood Pressure (BP) Screenings at sites around Vanderburgh County.  View calendar: monthly, yearly.
  2. Follow-Up as needed for:
        •    BP elevations at previous screenings
        •    monitoring BP following new medication treatment
        •    referrals from primary care providers
  3. Education and counseling:  General information given related to the prevention and management of hypertension including:
        •    lifestyle modifications 
        •    goal setting, including weight guidelines.

        •    recommended guidelines for BP measurements of adults over the age of 18
        •    home blood pressure monitoring guidelines 
  4. Referrals:  Will make necessary referrals to doctors, healthcare clinics, other community agencies, public health nurses or classes.

 BLOOD PRESSURE ASSESSMENT CERTIFICATION:

Training is available for health care providers on current BP guidelines, BP technique, and BP devices, to establish a standard of proficiency in Vanderburgh County.  Call 812-435-5015 to schedule.
