The objective of the Chronic Disease Program is to reduce the incidence of disease and death in Vanderburgh County related to hypertension, cardiovascular disease, diabetes, cancer, and renal disease.
SERVICES PROVIDED (all free) INCLUDE:
BLOOD PRESSURE ASSESSMENT CERTIFICATION

Blood Pressure (BP) Screenings at sites around Vanderburgh County. View calendar: monthly
Follow-Up as needed for:
• BP elevations at previous screenings
• monitoring BP following new medication treatment
• referrals from primary care providers
Referrals: Will make necessary referrals to doctors, healthcare clinics, other community agencies, public health nurses or classes.
Fasting blood glucose screening in our office by appointment (812-435-5015), with referrals to doctor as needed. View fasting blood glucose ranges
Education and counseling: General information given related to the prevention and management of diabetes including:
• Diabetes risk assessment quiz
with prevention information packet
• Physical activity packet with walking booklet and step meter to promote exercise
• Standards of care guidelines
We are available to participate in community health fairs, and we offer presentations on the management and prevention of chronic disease for public meetings. Topics could include information on diet, weight, physical activity, medications, smoking cessation and recommended guidelines for health screenings.
